• A Department of Defense program that works with civilian and military agencies to provide coordinated comprehensive support services to Families with special needs.

Enrollment:

• Who is eligible for EFMP? - All military members.
• Enrollment in EFMP allows you to have access to Extended Care Health Option (ECHO) through TRICARE. ECHO eligibility is for service members on orders for AGR, Title 10, and ADOS.

To enroll, please download and fill out form (DA 7415).  Have your exceptional family member’s doctor fill out form (DA 2792).  Submit both forms to FT Carson for processing:
