IMPORTANT COVID-19 UPDATE

Public walk-ins to the DEERS office will be allowed but will not disrupt appointments. All customers will be required to wear a mask at all times while in the building to reduce the risk of COVID-19 transmission. 

Appointments can be scheduled only for urgent or mission-critical needs and only for service members and dependents if it is relating to benefits, CAC PIN reset, or replacement CAC card that is needed for work.

For DEERs updates, such as changes to dependent status, or orders updates, current service members should forward all supporting documents to their Unit Administrator, or Unit Readiness NCO for processing. Current CAC card holders may use the DOD website to temporarily extend their expiration date, and CAC certificates until June 30, 2021.

A temporary extension to the expiration date of Retiree and Dependent USIDs that may expire on or after January 1, 2020, has been implemented by the DOD extending the expiration dates to June 30, 2021. NOTE: A service member with a valid CAC can create a DD 1172 in order for their Family Members to receive a new USID at Draper. The DD 1172 will be saved to the Family Member’s profile.

ALL PERSONS 18 or older wishing to receive a Military USID, and anyone wishing to receive a CAC, must bring their old CAC or USID card and two forms of unexpired government issued ID. If the old CAC or USID is unexpired, it may count as one of the two forms of unexpired government issued ID. Acceptable forms of unexpired government issued ID include: Military USID, CAC, Driver License (US or Canadian), State ID, Passport (any country), Passport Card, Concealed Weapon Permit, Voter Registration Card, Social Security Card, Birth Certificate (if the Birth Certificate is not in English, a verified translation into English must be provided).

SERVICE MEMBERS must bring their old military ID or CAC card, and two forms of unexpired government issued ID.

RETIRED MILITARY PERSONNEL, if it is their first visit to a DEERS facility since retiring, must bring a copy of their retirement order, 20 year retirement letter, and DD214/NGB 22. Retired personnel must bring their old retired USID, and two forms of unexpired government issued ID.

100% DISABLED VETERANS, if it is their first visit to a DEERS facility since receiving disability status, must bring a letter from the VA stating entitlement for a Disabled Veteran ID from DEERS. 100% Disabled Veterans must bring their old USID card, and two forms of unexpired government issued ID.

MEDICALLY RETIRED SERVICE MEMBERS, if it is their visit to a DEERS facility since receiving a Medical Retirement, must bring their Medical Retirement Orders and DD214 or NGB 22. Medically Retired service members must also bring their old USID or CAC, and two forms of unexpired government issued ID.

FAMILY MEMBERS must bring a DD 1172 signed by the sponsor and a DEERS official within the last 90 days. The DD 1172 must explicitly request issuance of a Family Member ID. A service member with a valid CAC can create a DD 1172 for their family member and enter the DD 1172 into DEERS at . A Family Member may provide a suitable, unexpired Power of Attorney to sign a DD 1172 in the Sponsor’s stead. Family members must bring their old Family Member USID card and Family Members over 18 must bring two forms of unexpired government issued ID.

FAMILY MEMBERS AGE 21-23 ENROLLED AS FULL TIME STUDENTS must bring a DD 1172 signed by the sponsor and a DEERS official within the last 90 days. The DD 1172 must explicitly request issuance of a Family Member ID. A service member with a valid CAC can create a DD 1172 for their family member and enter the DD 1172 into DEERS at . In addition to the requirements for FAMILY MEMBERS above, the Student must bring a letter from the University Registrar’s office with seal and signature stating the Student is enrolled full-time.

QUALIFYING FORMER SPOUSES/UN-REMARRIED FORMER SPOUSES AND DESIGNATED BENEFICIARIES will sign a DD 1172 as their own Sponsor at the DEERS facility, and must also bring all other documents noted for FAMILY MEMBERS above.

OCONUS CONTRACTORS must bring their Letter of Authorization (LOA) and two forms of government issued ID.

ALL OTHERS, including Federal Employees, Federal Retirees, Eligible Contractors, and Eligible State Employees must bring two forms of government issued ID.

Children are eligible for an ID on their 10th birthday. However, if your child appears older than 10, or the child’s parents aren’t married to each other, the child is eligible for an ID. Please bring a copy of the child’s Birth Certificate and Social Security Card.

What you wear for your ID photo is branch specific. All service members must be compliant with their branch of service grooming standards in order to receive an ID. All Marines must be in uniform in order to receive a CAC. Marines that do not have a blouse can have one provided for them by Draper DEERS staff.

FAMILY MEMBERS – ADD/REMOVE/CHANGE

NOTE: It takes up to 24 hours for TRICARE to see a new family member added to DEERS.

If you are a military service member, first provide copies of the below required paperwork to your ADMIN section. For military service members, retired service members, 100% disabled veterans, or other Designated Beneficiaries, you can add an unmarried child, step-child, adopted child, or ward to DEERS. We need a birth certificate, certified adoption certificate, or court order of guardianship in order to add a child to DEERS. A Social Security Card number is helpful, but not required for newborns. If you are a military service member, first provide copies of the below required paperwork to your ADMIN section. For military service members, retired service members, 100% disabled veterans, or other Designated Beneficiaries, you must bring your sponsor ID, certified marriage certificate, and a copy of your spouse’s valid ID and Social Security Card. 

If you are a military service member, first provide copies of the below required paperwork to your ADMIN section. For military service members, retired service members, 100% disabled veterans, or other Designated Beneficiaries, you must bring a certified Divorce Decree. Your former spouse and any step-children not adopted by the sponsor will be removed from DEERS and TRICARE eligibility. Turn in your former spouse’s and associated step-children’s Dependent ID card(s), or advise them to do so.  

Parents/Parents-in-Law, Wards of the Court, or Unmarried children over 21 incapable of self-support may be added to DEERS and eligible for TRICARE based upon a Favorable Determination from Defense Financing and Accounting Services (DFAS). In order to begin the process, please go to https://www.dfas.mil/militarymembers/SecondaryDependency/SDC.html   

If DFAS awards the case with a Favorable Determination, they will provide a letter for DEERS to enroll your family member. The family member must bring a DD 1172 signed by the sponsor and a DEERS official within the last 90 days, the Favorable Determination Letter from DFAS, and two forms of unexpired government issued ID. UPLOADING ORDERS/CHANGE IN ORDERS

First, verify with your ADMIN section that your orders have been properly submitted. If your ADMIN section instructs you to turn in the orders to the DEERS station, then do so and DEERS staff will verify that your orders have been uploaded.

IMPORTANT: Every time a Reservist or National Guard member receives a change in orders, they should verify their and their family’s TRICARE eligibility here: or call TRICARE CUSTOMER SERVICE 844-866-9378. It takes up to 24 hours from being loaded in DEERS for TRICARE to see a change in you or your family members’ status.

LOST/STOLEN/DAMAGED CAC REPLACEMENT

CAC holders are required to submit documentation from the appropriate security office or CAC sponsor confirming that the CAC has been reported lost or stolen (DoDM 1000.13-M-V1, encl. 2, subpar. 5.c.(3)). The document should be a Memorandum for Record (MFR), with appropriate seal and letterhead of security office or sponsor unit, with the subject line: “SUBJ: Memorandum for Record, Lost or Stolen CAC.” Please ensure the name of the individual, date, place and circumstances of the loss or theft are included in the statement, and that the statement includes the individual has been appropriately counseled on the importance of maintaining positive control of a CAC. The statement must be signed (CAC digital signature OK) by an appropriate representative of the security office or sponsor unit. The statement will be scanned and stored in your DEERS record.

If your CAC was confiscated and the confiscating authority provided you a notice of confiscation, bring that notice to the DEERS office. If the confiscating authority did not provide you with a notice of confiscation, you must bring a Memorandum for Record (MFR) for a confiscated CAC to the DEERS facility, with appropriate seal and letterhead of your security office or sponsor unit. The MFR must have the subject line: “SUBJ: Memorandum for Record, Confiscated CAC.” Please ensure the name of the individual, date, place and circumstances of the confiscation are included in the statement, and that the statement includes that the individual has been appropriately counseled on the importance of complying with CAC holder responsibilities. The statement must be signed (CAC digital signature OK) by an appropriate representative of the security office or sponsor unit. The statement will be scanned and stored in your DEERS record.

Two forms of identification (one must be a valid photo ID) are required to receive a replacement CAC Card for ALL lost, stolen, or confiscated CAC Cards. If the lost or stolen CAC is found after a new CAC is made, please return the old CAC for destruction at any DEERS ID facility.

TRICARE/DENTAL/VISION/PRESCRIPTIONS

It takes up to 24 hours for TRICARE to see a change in DEERS status.

If you are over 65 and entitled to TRICARE, you must enroll in Medicare Part A and Medicare Part B to be eligible for TRICARE FOR LIFE. Go to www.tricare4u.com or call 866-773-0404 for TRICARE FOR LIFE information and eligibility.

If your dependent child is attending college/university full time and is 21 but not yet 23, they are eligible to stay on TRICARE as long as they maintain full time status. In order to be eligible for TRICARE and to receive a Family Member ID, the child must provide a DD 1172 signed by the sponsor and a DEERS official within the last 90 days, a signed letter from the school’s REGISTRAR’S office, and provide 2 forms of unexpired government issued ID. Unmarried children 21 or older not attending college/university full time, or unmarried children 23 or older may sign up for TRICARE YOUNG ADULT at www.tricare-west.com or call 844-866-9378. The child is eligible for TRICARE YOUNG ADULT until the day before their 26th birthday. The child must be enrolled in TRICARE YOUNG ADULT before an ID card can be issued at a DEERS facility. To receive a Family Member ID that will function as a TRICARE YOUNG ADULT health care card, the child must provide a DD 1172 signed by the sponsor and a DEERS official within the last 90 days, and provide 2 forms of unexpired government issued ID. DEATH OF A SPONSOR OR FAMILY MEMBER

DEERS requires a certified copy of the Death Certificate. The certified copy will be scanned and returned to you. Please make an appointment at https://rapids-appointments.dmdc.osd.mil and bring the Death Certificate and 2 forms of unexpired government issued ID. New USID’s will be issued as needed, and the DEERS staff will assist you with starting the Survivor Benefits process.
