Mission Statement: Plan, program, provide and sustain health force protection and medical/dental support to ensure medical readiness, operations, training, mobilization, and demobilization of Utah Army National Guard units. Maintain and manage electronic healthcare records. Oversee the transfer/transition of Soldier healthcare information between units, civilian medical organizations and federal agencies. Medical advisory to JFHQ and TAG. Supports Medical Readiness for the UTANG.

It is an individual Soldier and Unit responsibility to ensure anyone coming to MedDet is in some sort of duty status for that day. MedDet is not responsible for ensuring duty status of the individual. 

If this is a PHA, the online portion of your Periodic Health Assessment, or PHA Survey must be completed on a government computer before your appointment. See below section for detailed instructions on how to fill out the online portion. 

All services require the Soldier to come wearing APFU (preferred) or OCP uniform. Any Soldier that arrives out of uniform (without prior authorization) may be asked to reschedule on a different day.

Online portion of PHA, or PHA Survey:

The online portion of the PHA can only be filled out on a government computer. From a government computer, navigate to the Medical Readiness Portal here: https://medpros.mods.army.mil/portal/

The Medical Readiness Portal can also be accessed on AKO under "My Cards"

At the bottom of the Medical Readiness Portal page, under "Self-Service" select Periodic Health Assessment

Under the Medical Health Assessments banner, select the button for "Start New Survey"

The Survey must be completed prior to your PHA appointment.

 

Periodic Health Assessment:

All appointments for periodic health assessments (PHAs) require completion of the online portion prior to the appointment start time. If you have any new medical documentation that needs to be reviewed by the provider, please bring that with you.

See the section  above for detailed instructions on filling out the online portion of the PHA, also known as the PHA Survey.

FASTING: All Soldiers age 40 and over will be screened for their cardiovascular risk percentage and are required to fast for 12 hours prior to their appointment. This means no food or drinks other than water!

HEARING: You will receive a hearing test during your appointment. Please refrain from exposure to any loud sounds for 14 hours prior to your appointment. Loud noises can temporarily alter your hearing and if your results are too different from your previous hearing tests, this will result in you needing to come back to MEDDET for up to two additional hearing tests (back to back) for confirmation of the shift in hearing. This must happen no sooner than the next day but no later than 90 days after the shift was detected.

DENTAL: Dental screening will be done at your PHA unless you bring a properly completed DD form 2813, found here: https://www.esd.whs.mil/Portals/54/Documents/DD/forms/dd/dd2813.pdf

The DD form 2813 must be dated within 6 months of your appointment. Your dental readiness classification assigned by your civilian dentist will be input and will stay valid for 1 year from the date on the form.

VISION: If you wear glasses, please bring them with you to your appointment. You will have both your corrected and
uncorrected vision screened.

 

Physical Exams for Chapter 2 Commissioning, Chapter 3 Retirement or Chapter 5 Special Schools:

You will fill out a large packet on the morning of your appointment. Part of the packet is a DD form 2807 - Report of Medical History. You may fill this form out prior to your arrival as this is the form that usually takes the longest to fill out. 

All Soldiers conducting physical exams must bring any documentation relevant to their exam with them. Additionally, the provider has the authority to request any documentation that they see relevant from the Soldier.


FASTING: Regardless of age, you are required to fast for 12 hours prior to your appointment. Please continue to drink plenty of water (ONLY water) during your fast!

DENTAL: Dental screening will be completed for Chapter 5 physicals (other than Ranger and Airborne) unless you bring a
properly completed DD form 2813 with you. These forms can be found

DD form 2813 must be dated within 6 months of your appointment. Your dental readiness classification assigned by your civilian dentist will be input and will stay valid for 1 year from the date on the form.

VISION: If you wear glasses, please bring them with you to your appointment. You will have both your corrected and uncorrected vision screened. If you have ever had LASIK/LASEK/PRK, you will need to bring your pre and post operation notes with you to your appointment.

COMPLETION OF YOUR PHYSICAL PAPERWORK WILL NOT SAME DAY AS YOUR APPOINTMENT. THEY CAN TAKE UP TO 8 WEEKS TO PROCESS. PLEASE PLAN ACCORDINGLY FOR COMMISSIONING OR SCHOOL DATES!

*If you are due for a PHA, this can be completed in the same appointment. Please let the staff know if you would like to do both physical and PHA. It is important to understand that the physical itself WILL NOT reflect as your annual PHA and is not completed in the same system.
